| In consideration of the furtherance of your purpose, objectives and work, and in consideration of your permitting me to participate in your event on behalf of myself, my heirs, executers, administrators and assigns, I hereby waive and release any and all rights and claim for damages which I may have against the Merrick Festival, the munic8ipality, itís sponsors or any personnel, their heirs, executers, administrators, successors and assigns for any and all injuries I may suffer from while taking part in this event or as a result thereof. Further I hereby grant full permission to any and all foregoing to use photographs, videotapes, motion pictures, recordings or any other record of this event for any purpose deemed necessary by The Festival. No one may enter this event without signing this official waiver. Sumbitting this form will have the effect of placing your signature on the registration form. |
